PARENTAL WAIVER AND RELEASE FOR ATHLETES OF MINORITY AGE

In consideration of being allowed to participate in Northern Kentucky Lacrosse Club (NKYLAX) games, practices, tournaments, clinics and meetings, I certify, as the parent/guardian of this participant, that I consent to the provisions of medical, psychological or psychiatric care and treatment, emergency medical services, transportation, housing and meals associated with such participation in programs conducted by NKYLAX. I represent that I am not aware of any physical condition or medical condition that prevents this participant from participating in the sport of lacrosse. In the event that emergency medical services are required, I hereby authorize NKYLAX to act to resolve such emergency without first obtaining my prior consent or the consent of the participant’s next of kin or any other individual. I understand that lacrosse can be a dangerous game and am aware of and accept the risk of injury, which includes bodily injury, damage to personal property, or death. I agree to release, indemnify, defend, and hold harmless NKYLAX and its affiliates, chapters, employees, volunteers, and members from any claims, liability, suits, or damages that my child or ward may have as a result of my child/ward participating in this lacrosse event.

I grant NKYLAX the right to photograph and/or videotape my child or ward and further display and use my/and our child or ward’s name, face, likeness, voice and appearance as deemed appropriate in all media (known or hereafter) in perpetuity. I understand that all photographs captured of me and/or my child or ward by NKYLAX staff and/or officially contracted event photographers / videographers will forever be the property of Raiders Lacrosse and may be used as deemed appropriate by NKYLAX to include, but not limited to: event / organizational publications, training/educational materials, websites, promotional materials and/or advertisements.

I am aware that NKYLAX does not offer refunds on fees once registration is submitted. Only extenuating circumstances will be discussed via the board on a case by case basis.
